In addition to being a Part-Owner of Senior Transition Experts, LLC, Kathy is a highly accomplished and results–driven Licensed Registered Nurse (RN), Case Manager, and nationally Certified Elder Care Coordinator (CECC). She brings to the table a strong management background and extensive experience transitioning seniors.
She has planned and coordinated the care of the elderly and others with high acuity needs, high dependency, unstable support systems, and those going through life transitions while coping with change or loss.
Kathy is highly skilled at assessing, planning, implementing and coordinating services to meet our clients’ needs using communication and available resources to promote quality cost-effective and stress free outcomes.
She has a proven history of coordinating transitions across the entire continuum –collaborating with clients, family members and, where necessary, medical and healthcare providers to seamlessly manage the entire transition process.
